Participation

To compete in any sport, athletes/parents must complete the Athletics Online Forms, via Skyward and the WIAA Sports Physical Form.

Athletics Online Registration - Families must fill out these forms once a year for each student.  You may call or email the Athletics Office (920) 848-4467 Email if you have any questions.

WIAA Sports Physical Form - A sports physical form needs to be completed every two years and a sports alternate card needs to be filled out every other year
