Attention 4K & 5K families for the 2025-2026 school year!

Attached, you will find our Required 4K & 5K Health Packet. These packets' contain important forms that need to be completed for your child's health and safety while at school. Please complete all the enclosed healthcare appointments and forms in order to return them to the school's office as soon as possible. Your prompt attention to this matter will ensure that we will have all the necessary information on file for your child, before the school year begins.

4K enrollment is now open for the 2025 - 2026 school year! Our 4 year old kindergarten program is a one year program that precedes 5 year old kindergarten. To be eligible for 4K, a child must be 4 on or before September 1, 2025. If you need any assistance with the 4K enrollment process please contact our office at (920) 848-4476, option 0, or email the office at ofesoffice@of-ps.org. To ensure consideration of your top two 4K program preferences, please submit your 4K application by February 28, 2025.
